We do require a referral letter from your family physician outlining your medical concern and your prior medical history. Our internal medicine physician will review your medical history and your medication list to provide a comprehensive analysis and devise a therapeutic plan. Kindly ensure that you bring your updated medication list to your appointment.

We do require all our patients to complete a COVID-screening questionnaire before their arrival for the appointment.
